1. We are so grateful to have you be a part of our program! Please do not hesitate to call us if you have any questions along the way.

3. You will need your child's Baptismal Certificate (if they have been baptized) to complete this form UNLESS you are POSITIVE you uploaded it last year during registration.
4. Christ the King School families are more than welcome to attend the Faith Formation program, but please note that both the school curriculum and culture provide for your children's religious education.
